Most likely something no one has to worry about anymore. I think it just refers to tracks being one after the other and in the right order. Pretty much every burner nowadays supports this AND Disc at Once (DAO).

When you burn a disk using TAO, there is a gap of silence (usually 2 seconds) between each file. Burning a disk using DAO (disk at once) eliminates the gap and gives you uninterupted music from track to track.....a very important thing when you are listening to a live show.
Disks burned for B&P's should always be burned DAO...but like Speen said TOA is rarely used.....if it is it is usually by accident.
