Most established bands use IEMs for a number of reasons--they are excellent. You can customize your mix and hear every band member better than using floor monitors, easier to control feedback, and musicians wont get hearing damage. Monitor engineers also pump in crowd tracks (that is why bands have small mics on both sides of the stage facing the crowd) so the musicians don't feel isolated. It's very rare to see a touring performer NOT wearing IEMs.
As for the metronome, I've heard of bands pumping in a click track into the IEM of the drummer so he can keep the beat...this is usually not the case for an established band, as their drummer is good enough to keep time. But other bands, such as Keane (who has a prerecorded bass track), has to use a click track so that the prerecorded bass track can "play along" with the live band.
