I sounded sooooooo bad 4 years ago. How did I do it? I kept singing and singing. Just like getting in shape, you just keep working out, and treating your body right. So, when you sing, try not to drink any syrupy drinks, like sodas (expecially colas), and some liquors. Acidy, like things with lemons, oranges, and such, help clean them. Even gargling salt water helps (don't drink it, just gargle it, and try to get it lower but not low enough to start to choke). Also, just working on songs, but if you chords get really sore, or tiring, or aren't working after a while of sinigng, that means take a break.
Just treat your vocal chords like how you would work out in a gym. It's another mucsle, treat it like one, with care, and it will help you in achieving a good singing voice. Also, try to sing from your gut, not your nose (unless you want that sound).

I can't believe that we would lie in our graves,
dreaming of things we might have been.
