Stay transition

Can someone help me out with when to play the transition and when to play the verse? It sounds to me like he plays the transition once after the chorus before going back to the verse, but I can't tell for sure. At the end, when it keeps repeating the last lines, is the verse or the transition riff playing? If someone could post the structure of the song (i.e. intro>verse>chorus>transition etc) I would greatly appreciate it.
